Concern about a minister's safeguarding practice

This form can be used to submit concerns about the safeguarding practice of an Accredited Minister, Recognised Local Minister, Nationally Recognised Pastor, or those training for these roles.

If the minister you are concerned about is minister of the church you yourself attend, please do not complete this form. Rather,  you should speak as soon as you can to the church's designated person for safeguarding (the 'church DPS'), if practical. They can pass the matter on to the regional safeguarding lead and, through them, the BUGB National Safeguarding Team.

If, alternatively, your concern is about the conduct of a minister but does not relate to safeguarding, please complete and submit our 'Complaint about a minister's conduct' form.

PLEASE NOTE: This form is in editable PDF format and you will need to open the form using Adobe Acrobat Reader. If you do not have this then you can download it for free here. Though you can read it in a web browser, please download it before you complete it.
