Complaint about a Minister's conduct

This form can be used to submit complaints of misconduct by an Accredited Minister, Recognised Local Minister, Nationally Recognised Pastor, or those training for these roles.

If your complaint relates in particular to the safeguarding practice of a minister rather than their general conduct, please do not fill in this form. Please instead speak if practical to your church's designated person for safeguarding ('the church DPS') if you attend the minister's church. If you do not attend the minister's church, please complete and submit our 'Concern about a minister's safeguarding practice' form.
